Yes, citations are added to Medline after they have been assigned Medical Subject Headings (MeSH).
From Fact Sheet What's the Difference Between MEDLINE® and PubMed®?
"In addition to MEDLINE citations, PubMed also contains:
* In-process citations which provide a record for an article before it is indexed with MeSH and added to MEDLINE or converted to out-of-scope status."
What I cannot find record of is what fields are searched in a simple keyword search in Pubmed. I assume that Author, Title, Journal Title, and Abstract fields are searched, but I haven't yet found out for certain.
